How to Refer

Fax either of the following to 541-319-8968.

1. Most recent encounter note with all of the patient’s current dx.
2. Fill out a referral form marking all of the patient’s current dx. We can also provide a referral form template if needed.

Insurance companies almost always require a referral form filled out by either an MD, DO, PA, or NP for continued medical nutrition therapy coverage. It is important to have all of the patient’s dx listed, included any of overweight or obesity.

Medicare will only accept a dx of Type 1 diabetes, Type 2 diabetes, or renal disease for medical nutrition therapy – max 3 hours a year.
